Thank you for your message.
Unfortunately, there are indeed some actions that may break the Google Drive integration.
To fix this, please reset the integration, by deleting it(not only disabling it) from your form, clearing the cache and cookies of your browser as far back in time as possible, and adding it back. By following these steps, when the new entry is received, a new spreadsheet should be created.

Form isn’t saving entries to Google Sheets anymore. It was working, but we changed some fields, so I deleted the Google Sheet hoping it would re-create it when the next form entry occurred – it didn’t. So I disconnected/reconnected the integration, but that didn’t help either 🙁
